To help make your first visit to Rialto Animal Hospital as smooth as possible, we’ve created this checklist of records and information to bring with you. Whether you’re visiting us from Rialto, Fontana, Rancho Cucamonga, Highland, Bloomington, Redlands, San Bernardino, or the surrounding communities, having these items ready helps our team provide the best possible care for your pet.
Filling out forms ahead of time means more time with our team — and less time at the front desk when you arrive. If you’re a new client (or just updating your info), please complete the appropriate form below and bring it with you, or email it to us at rahrecep@gmail.com before your appointment.

What to Bring to Your First Visit
To help us provide the best possible care and avoid delays, please bring any of the following information that applies to your pet:
- Previous medical records
- Vaccination history
- Previous parasite testing (fecal) results
- Deworming history
- Spay/neuter certificate or adoption paperwork
- A list of any current medications and supplements
- Information about your pet’s current diet and treats
- Pet insurance policy information (if applicable)
- Any recent diagnostic test results, radiographs (X-rays), or specialist records
- Your completed New Client and Patient Form and/or Boarding Intake Form
Don’t worry if you don’t have everything—we’re happy to help gather records from your previous veterinarian whenever possible.
